Register a player with the contact details a club actually needs, and a place to add the declarations your own governing body requires.
Who it is for
Sports clubs, junior teams, leagues, holiday camps and activity groups.
How to use it
- Take at least two emergency contacts, with a note on who may collect a child.
- Ask what a coach must know to run a session safely — not for a medical history.
- Write your governing body's or insurer's declarations into the space provided.
- Have a parent or guardian sign for anyone under 18.
- Store completed forms securely and review them each season.

Contact details, not medical records. This form asks where medical information is held and who to call. It prints no medical fields, no condition list and no treatment, and it is not a consent form, a waiver or a disclaimer — those are legal documents whose wording and effect differ by country, and your club's governing body or insurer will tell you what yours must say. This sheet holds personal information. Keep it somewhere secure, share it only with people who need it, and keep it only as long as you need it. Rules on holding personal data — especially about children — differ by country and apply to paper as much as to computers.

Why does the form not have medical or consent sections?
Because a consent form, a waiver or a medical declaration is a legal document whose wording and effect differ by country, and a printable that pretended to supply one could leave a club with nothing when it mattered. Your governing body or insurer will tell you what yours must say — write it in.
