What this sheet is for
Register a player with the contact details a club actually needs, and a place to add the declarations your own governing body requires.
Who it is for
Sports clubs, junior teams, leagues, holiday camps and activity groups.
How to use it
- Take at least two emergency contacts, with a note on who may collect a child.
- Ask what a coach must know to run a session safely — not for a medical history.
- Write your governing body's or insurer's declarations into the space provided.
- Have a parent or guardian sign for anyone under 18.
- Store completed forms securely and review them each season.
What is on the sheet
The layout is fixed at a size that fits both A4 and US Letter, so the A4 and Letter files are visually identical and print at full size on either. Rows are set at a height that an adult can actually write a date and a few words into with a pen — most printable tables are built in a word processor and never tested with one.
Questions
Why does the form not have medical or consent sections?
Because a consent form, a waiver or a medical declaration is a legal document whose wording and effect differ by country, and a printable that pretended to supply one could leave a club with nothing when it mattered. Your governing body or insurer will tell you what yours must say — write it in.
