What this sheet is for

Have paper proof of delivery for a whole round when the handheld fails or the customer insists on signing something, in a form that will stand up to a claim.

Who it is for

Parcel and multi-drop couriers, self-employed drivers on a delivery contract, and small businesses delivering their own orders.

How to use it

  1. Fill in the driver, round, vehicle and date; write the parcel count out at the foot before you leave.
  2. At each drop, write the time, the tracking number and the address.
  3. Record exactly who took it or exactly where it was left — a neighbour's number, the named safe place.
  4. Get a printed name as well as a signature.
  5. Tick the ID column where the sender requires a check, and total the round at the end.

What is on the sheet

One page, top to bottom:

Printed under the title: A paper proof of delivery for a round — who each parcel was handed to, or where it was left and with whom — for when the handheld is down or the customer wants ink.

Legible name, exact place. A signature is only proof with a legible name and a time beside it. For anything left with a neighbour or in a safe place, write exactly where — 'left safe' alone is what a claim turns on. ID or age checks are whatever the sender requires.

Questions

When do I need an ID or age check?

Whenever the sender's terms or the rules where you are say so for that item — the sheet cannot know which parcels those are. Tick the column when you did one, and note what you checked in the name column if your contract wants it.
