What this sheet is for

Record every check on every dog with the time and who made it. A dog that has not eaten or has a loose stool is then passed on the same day.

Who it is for

Boarding kennel, rescue and day care staff on morning, evening and night duty.

How to use it

  1. Write each kennel and dog down the side at the start of the day.
  2. At each check, write the time and your initials in that check's pair of columns.
  3. Note eating, drinking, urine and faeces as you see them.
  4. Write anything not right in plain words, with who you told and when.
  5. Record the out-of-hours check as seen in person or on camera, then have the manager sign the sheet.

What is on the sheet

One page, top to bottom:

Printed under the title: One day, one block: each dog's morning and evening check with the time and who did it, eating, drinking and toileting, the out-of-hours check and anything that was not right, with who was told.

Seen, written, passed on. Write what you saw, not what it might mean: a dog that has not eaten, a loose stool or a limp is noted here and passed on as your own procedure says, and the vet decides what it means.

Write-on lines: Date · Block · Morning staff · Evening staff · Out-of-hours

Checks — one line per kennel; out of hours: P in person, C camera

Also printed: Morning · Evening · Out of hours

Table, 12 rows: Kennel · Dog · Time · By · Ate · Drank · Urine · Faeces · Time · By · Time · How · Not right · Action

Write-on lines: Dogs seen by the vet today, and why · Manager's check: time · Signed

Questions

Why no list of symptoms to look out for?

Because the sheet is a record of what was seen, not a diagnosis. Write what you noticed in plain words and pass it on as your procedure says; your vet decides what it means.

Do we need an out-of-hours check?

That depends on your licence conditions and standards, so check the rules where you are. If you make one, the sheet records when, by whom and whether it was in person or by camera.
