Puppy Vaccination and Worming Schedule

What this sheet is for

Keep the first year of a puppy's treatments in one place, with the dates your vet gave you written down before you forget them. The weight at each treatment is worth recording too, because a puppy's changes week to week and nobody remembers it afterwards.

Who it is for

New puppy owners, breeders handing a litter over, and anyone taking on a young dog with no paperwork.

How to use it

1. Ask the vet at the first appointment for the plan they recommend, and write their dates straight into the 'due' column.
2. Weigh the puppy before each treatment and record the weight on the same line.
3. Write the product name and the batch or lot number as they appear on the packet.
4. Fill in the next due date before you leave the practice, while somebody can still tell you what it is.

What is on the sheet

The layout is fixed at a size that fits both A4 and US Letter, so the A4 and Letter files are visually identical and print at full size on either. Rows are set at a height that an adult can actually write a date and a few words into with a pen — most printable tables are built in a word processor and never tested with one.

Questions

Why does this sheet not print a schedule?

Because a puppy schedule is not universal. Which treatments are given, at what age, with what product and how far apart differs by country, by region and by the individual dog. Printing one here would be guessing on your behalf, so the column is blank and your vet fills it in.

What should I do if I miss a date?

Ring the practice rather than working it out yourself, and write down what they tell you to do next. Note the missed date on the sheet too — it is the sort of detail that gets forgotten and then matters.

The breeder gave me a card. Do I still need this?

Keep the card, and copy it onto this sheet. Breeder cards are usually partial, often lose the batch numbers, and are the first thing to go missing in the first month.
