Cat Health Record

What this sheet is for

Hold a cat's identification, insurance and treatment history on one page that a cattery, a sitter or a new practice can read. It has room for the things that are specific to cats and get left off general pet forms.

Who it is for

Cat owners, fosterers, and anyone taking a cat to a new vet or a cattery.

How to use it

1. Fill in the identification block once, including the microchip number and the database the chip is registered with.
2. Record whether the cat goes outside, and where it hides — a sitter will need both.
3. Copy each vaccination or treatment from the practice's record, with the batch or lot number.
4. Use the last block for ongoing conditions, written plainly enough for a stranger to follow.

What is on the sheet

The layout is fixed at a size that fits both A4 and US Letter, so the A4 and Letter files are visually identical and print at full size on either. Rows are set at a height that an adult can actually write a date and a few words into with a pen — most printable tables are built in a word processor and never tested with one.

Questions

Why is there a space for FIV and FeLV results?

Because a new vet, a cattery or a rescue may ask, and the date and result are easy to lose between practices. The sheet records only what your own practice reported — whether testing is worthwhile for your cat is a question for them.

Does an indoor cat need a different record?

The sheet is the same, but what goes in it may not be. Whether a cat goes outside is the sort of context anyone reading the record will want early, which is why there is a field for it near the top rather than buried in the notes.
