It differs between facilities and between countries, so ask the place you are booking and write their answer in the boarding section at the bottom. What they generally want is a dated record of what was given and when the next one falls due, which is what the table on this sheet holds.
No. The certificate is the document with the practice's stamp on it, and you should keep it. This is the copy you can write on, take with you and hand over without risking the original.
Because there is no single correct one. What a dog is given, and how often, depends on the dog, on where you live and on what your vet has seen locally. A printed schedule would go out of date and could be wrong for your animal, so the sheet stays blank and your vet fills the gaps.
Keep the first year of a puppy's treatments in one place, with the dates your vet gave you written down before you forget them. The weight at each treatment is worth recording too, because a puppy's changes week to week and nobody remembers it afterwards.
Put everything a neighbour, a sitter or an emergency responder would need onto one page and leave it where a stranger would look. It answers the questions that get asked when you are not there to be asked.
Keep one continuous record of everything that has been serviced, repaired or replaced, so the history of the building exists in one place rather than in a drawer of loose invoices.
