Track weight against what the animal is actually being fed, so a change in one can be read against the other. A single weighing tells you very little; a column of them taken the same way each time is something a vet can use.

The layout is fixed at a size that fits both A4 and US Letter, so the A4 and Letter files are visually identical and print at full size on either. Rows are set at a height that an adult can actually write a date and a few words into with a pen — most printable tables are built in a word processor and never tested with one.
That is a question for your vet, and the answer depends on the animal, its breed, its age and its build rather than on any table. There is a field near the top to write down the figure they give you, so the chart has something to be measured against.
Because in many households more than one person does, and a weight change nobody can account for is worth checking against who fed what. Writing it down settles the question without an argument.
Either, as long as it is the same one each time and you write down which you used. Comparing a reading from bathroom scales with one from the practice is how a trend turns into noise.
