What this sheet is for

Record volunteering as it happens, with a signature against each line, so the hours can be proved months later to somebody who was not there.

Who it is for

Volunteers, students logging community service, Duke of Edinburgh and award-scheme participants, charities and the people who supervise them.

How to use it

  1. Fill in a line at the end of each session, not at the end of the term.
  2. Describe what was actually done — 'helping' is not something anyone can verify.
  3. Get each line or each page signed at the time.
  4. Keep your own copy as well as the organisation's.

What is on the sheet

The layout is fixed at a size that fits both A4 and US Letter, so the A4 and Letter files are visually identical and print at full size on either. Rows are set at a height that an adult can actually write a date and a few words into with a pen — most printable tables are built in a word processor and never tested with one.

Questions

Will a school, university or employer accept this?

They set their own requirements and some issue their own form, so ask first. Where there is no set form, a dated record signed by a supervisor at the time is the thing they are usually asking for.

What if nobody was there to sign?

Write the hours down anyway with a note saying who can confirm them and how. A contemporaneous record with an honest gap is worth more than a tidy one filled in later.
