What this sheet is for

Record why a decision was taken and what else was on the table, which is the question people actually ask months later.

Who it is for

Project groups, committees, trustees, partnerships and anyone who will be asked to justify a choice after the fact.

How to use it

  1. Log the decision on the day it is taken.
  2. Write the reasoning in one or two plain sentences.
  3. Record the alternative you rejected and why — this is the column that earns its keep.
  4. Note whether and when the decision should be revisited.
  5. Cross-reference the minute number if the decision was taken in a meeting.

What is on the sheet

The layout is fixed at a size that fits both A4 and US Letter, so the A4 and Letter files are visually identical and print at full size on either. Rows are set at a height that an adult can actually write a date and a few words into with a pen — most printable tables are built in a word processor and never tested with one.
