What this sheet is for

Track the things that have already happened separately from the things that might, so today's problems do not crowd out next month's risks.

Who it is for

Project teams, site managers, facilities teams, committees and small businesses.

How to use it

  1. Raise the issue on the day, with the date and who raised it.
  2. Say what the impact is, not just what happened.
  3. Give it an owner. An unowned issue is a complaint.
  4. Record what was actually done and the date it closed.
  5. Move anything that has not happened yet to the risk register instead.

What is on the sheet

The layout is fixed at a size that fits both A4 and US Letter, so the A4 and Letter files are visually identical and print at full size on either. Rows are set at a height that an adult can actually write a date and a few words into with a pen — most printable tables are built in a word processor and never tested with one.
