Turn a scoring walk into a list of cows that were examined, treated and re-scored. The monthly trend then shows whether lameness in the herd is going up or down.
Dairy farmers, herd managers, foot trimmers and vets who score mobility regularly.
2 pages, in this order:
Printed under the title: One scoring session: the group walked, the tally at each score on the scale you use, then every cow in your action scores followed until she is re-scored.
Name your scale first. No scoring scale, action score or target is printed. Mobility is scored on different scales in different countries: write yours at the top, score in the same place and the same way each session, and agree with your vet which scores need action.
Write-on lines: Herd or holding · Date · Scorer · Group · Scale used · Action scores · Surface walked on · Cows scored
Table, 5 rows: Score · Tally · Cows · Share · Action for this score
Also printed: Cow · Score · Leg · Examined · What was found · Action · Re-score · Result · tag · date · vet, trimmer · trimmed, treated · score
Also printed: Cow · Score · Leg · Examined · What was found · Action · Re-score · Result · tag · date · vet, trimmer · trimmed, treated · score

Because there is more than one in use: some countries score on a four-point scale, others on five, and the descriptions differ. Name yours at the top and use only that one, so this month's tally can be compared with last month's.
The ones in the scores you and your vet have agreed need action. Write those scores in the box at the top, then list every cow scored in them, so none is scored and then forgotten.
The trimming record is what was done at each foot. This sheet is the herd-level screening that decides which cows go to the crush in the first place, and checks they improved afterwards.
Record foot problems by animal so that the repeat offenders become obvious, which is the single most useful thing a lameness record does.
Record condition scores consistently over a year, so changes are visible at the points in the cycle where they matter.
Record yields per animal daily, with the notes that turn an unexplained drop into an early warning.
Find the broken latch or loose race sheet before the cattle are in the race, not while they are. Each fault is written down with its repair and a sign-off.
