What this sheet is for

Settle 'that scratch was not there when you collected it' with a signed condition note taken before the vehicle went on the bed.

Who it is for

Recovery operators, garages that run their own flatbed, and mobile mechanics who bring cars in for customers.

How to use it

  1. Fill in the job number, who requested it and the recovery vehicle before you leave.
  2. On scene, record where the vehicle was and tick its condition before touching it.
  3. Note every existing mark in the damage table and get the customer to initial it.
  4. Record the times called, arrived, loaded and delivered, and who took the keys.
  5. Get a signature from whoever receives the vehicle at the other end.

What is on the sheet

One page, top to bottom:

Printed under the title: One sheet per recovery or tow — who called, where the vehicle was, what state it was in when you got there, how it was loaded and where it went — signed at both ends.

Condition on arrival, before loading. The condition block is the part that matters a week later. Note every existing scuff, the wheels that would not turn and the fuel gauge before the vehicle goes on the bed, and get the person on scene to sign it.

Questions

Why no mileage or call-out rate on the sheet?

Because the rate is yours and the sheet is not an invoice. Mileage out and back are recorded so you can bill from them using whatever rate you charge.

What if the driver or owner is not on scene?

Note that in the customer initials column and take photographs instead. The condition block is still worth filling in — it is your record even without a counter-signature.
