What this sheet is for

Turn a diagnostic session into a record the next technician can pick up. It also gives the customer something better than 'it had a code'.

Who it is for

Garages and diagnostic specialists, mobile technicians with a scan tool, and fleet workshops chasing intermittent faults.

How to use it

  1. Write the symptom in the customer's own words, and when it happens.
  2. Read every module and list each code with its status before clearing anything.
  3. Log each test or live-data reading with the specification from your data source beside it.
  4. Write the cause found, the repair done or advised, and whether codes came back after the road test.

What is on the sheet

One page, top to bottom:

Printed under the title: A diagnostic session written down: the symptom as the customer described it, every code read with its status, the tests that followed, and what was concluded — so the next person does not start again from the scan tool.

Write-on lines: Vehicle reg · Make / model / engine · Mileage · Job card · Date · Symptom as reported by customer · When it happens · Warning lights on

Codes read — every module — before anything is cleared

Table, 5 rows: Module · Code · Description · Status · Freeze frame notes · Cleared

Tests and live data — your reading, with the manufacturer's spec from your data source beside it — never guessed

Table, 4 rows: Test / parameter · Reading · Spec (from data) · Conditions · Result

Write-on lines: Conclusion — cause found · Repair done / advised · Time on diagnosis · Codes re-read after repair · Road tested · Technician · Customer told

Questions

Why is there no 'expected value' printed for anything?

Because the right figure depends on the engine, the sensor and the conditions when it was read. Your data source has it; the sheet has a column next to your reading so the two sit side by side.

Why record codes before clearing them?

Because a cleared code is gone, and with it the freeze-frame data that says what the car was doing when it set. The sheet asks for the list first and the clearing as a separate tick.
