Put the part numbers, the timing checks and the next-due figures on one sheet that goes in the customer's file, so a warranty query or a resale question has an answer.
Garages doing timing belt and major service work, and owners who want the record of the biggest service their car gets.
One page, top to bottom:
Printed under the title: The cambelt job and the major service done with it, for one vehicle: the parts that went on with their part numbers, the service items ticked, and when the next belt falls due by the manufacturer's interval.
Write-on lines: Vehicle reg · Make / model · Engine code · Mileage · Date · Customer · Job card · Technician · Previous belt (if known)
Table, 5 rows: Part · Part no. · Make / supplier · Fitted · Not fitted — why
Write-on lines: Next belt due — mileage (mfr schedule) · Or by date · Schedule source · Customer told
Ruled lines: Notes — condition of old belt, anything found, advisories
Because it depends on the engine, and sometimes on how the car is used. The manufacturer's schedule is the only figure worth writing, so the sheet gives you a box for it and asks where it came from.
That depends on whether the belt drives it and what the manufacturer says — the sheet has a column for it, and a column for why it was left, so the decision is on record either way.

Show a customer or a fleet manager the actual figures behind an advisory, and see at the next visit how much has worn since the last one.
Stop looking up the same specifications every time. Fill it in once from the handbook and keep it with the vehicle.
Capture a job while you are still standing in it: what was asked for, what was done, what went into it and how long it took.
