What this sheet is for

Settle the 'that mark was not there' conversation before it starts. Mileage and fuel level are proved at both ends of a collection.

Who it is for

Garages, body shops and dealers that collect and return customers' cars, and the driver who does it.

How to use it

  1. Fill in the collection half at the customer's address with the customer present.
  2. Note every existing mark, where it is, and that the customer was shown it.
  3. Get both signatures before the car moves.
  4. Fill in the delivery half on return, walk the customer round against the notes, and sign again.

What is on the sheet

One page, top to bottom:

Printed under the title: The two handovers that happen away from the counter — picking a car up from the customer and taking it back — with mileage, fuel, keys and condition agreed at each end.

Write-on lines: Job card no. · Vehicle reg · Make, model & colour · Driver · Customer · Collection address · Contact no.

Collection — at the customer's address — signed before the car moves

Write-on lines: Date & time · Mileage · Fuel level · Keys received (no.) · Warning lights on

Table, 4 rows: Existing damage / marks noted · Where on the vehicle · Customer aware

  • Items left in the vehicle listed on the job card and the customer told
  • Locking wheel nut key, service book and any spare key noted as received or not

Signature strip: Customer signature · Driver signature · Date

Delivery — back at the customer's address — signed at handback

Write-on lines: Date & time · Mileage · Fuel level · Keys returned (no.) · Invoice handed over

  • Work done explained; old parts handed over if asked for; advisories pointed out
  • Vehicle walked round with the customer against the collection notes above

Signature strip: Customer signature · Driver signature · Date

Ruled lines: Notes — items in vehicle, route, anything the customer said

Questions

Why is this separate from the check-in sheet?

Because the handover happens twice and away from the workshop, with a driver rather than a service adviser. The Vehicle Intake and Condition Record is for a car that arrives at the counter; this one travels with the driver and is signed at the customer's door.
