What this sheet is for

Make sure nobody is quietly missed for months, and let a visiting team hand over without anyone repeating the same conversation.

Who it is for

Clergy, pastoral teams, chaplains, befriending schemes and community groups who visit people at home, in hospital or in care.

How to use it

  1. Agree with your group what may be written down before you start using this.
  2. Record the visit itself: date, person, place and who went.
  3. Keep the purpose and action columns brief and factual.
  4. Note who anything was passed to, and only pass it on with consent.
  5. Put a date in the next-visit column so the round keeps turning.

What is on the sheet

One page, top to bottom:

Printed under the title: A plain record of visits made — who was seen, when, by whom, and what was agreed to happen next.

Your policy decides what is written. Keep this to the fact of the visit and what follows from it. What may be written down, who may read it and how long it is kept are set by your group's own confidentiality and safeguarding policy — follow that, and write nothing here it does not allow.

Write-on lines: Group / parish · Visitor or team · Period covered

Visits — one row per visit

Table, 13 rows: Date · Person visited · Where · Visited by · Purpose · Action agreed · Passed to · Next visit

Questions

How much detail should go in the action column?

Only what someone else in the team would need in order to follow it up. This is a log of visits, not a case file, and the less it holds the easier it is to keep it somewhere it belongs.

Why does the sheet not tell me what I may record?

Because that is set by your group's own confidentiality and safeguarding policy and by the data protection rules where you are. Follow those; this sheet is deliberately narrow so it fits inside most of them.
