What this sheet is for

Leave a sitter with the numbers, the address and the routine written down rather than explained in a hurry at the door.

Who it is for

Parents leaving children with a sitter, a relative or a friend for an evening.

How to use it

  1. Write the full address down. Nobody can recall it when they are frightened.
  2. Fill in the emergency services number for where you are.
  3. Go through the sheet in person before you leave, rather than pointing at it.
  4. Say where written instructions for allergies or medicines are kept — and agree beforehand what the sitter should and should not do.
  5. Leave it somewhere obvious, like the fridge.

What is on the sheet

The layout is fixed at a size that fits both A4 and US Letter, so the A4 and Letter files are visually identical and print at full size on either. Rows are set at a height that an adult can actually write a date and a few words into with a pen — most printable tables are built in a word processor and never tested with one.

Questions

Why does it not have a section for medicines and doses?

Because a dose written on a sheet by somebody in a hurry is exactly the kind of instruction that goes wrong. Keep the written instructions that came with the medicine, show the sitter where they are, and agree in advance what they should do — including when to call you or a doctor.
