Who this is for

One form for the club leader. It gathers contacts, collection, allergies, medicines and consents, so nothing is left to a phone call on the day.

Parents booking a child into an after-school club, holiday scheme, sports camp or activity day, and leaders of small clubs who have no form of their own to hand out.

What this helps you track

  • Who to ring first if something happens, and how to reach them
  • Which adults the child may go home with
  • Conditions and allergies the leader needs to know about
  • Medicines the child brings, the dose as prescribed and who gives it
  • Which activities, photos and journeys the parent agrees to

How to use it

  1. Fill in the child's details, the club or activity and the dates the form covers.
  2. List the emergency contacts in the order they should be rung, and tick the ones who may also collect the child.
  3. Write down any condition or allergy, the signs to watch for and what the leader should do.
  4. Copy each medicine exactly as the label or prescription gives it, and say who gives it and when.
  5. Tick only the consents you are happy to give, then sign and date the form.
  6. Print a fresh copy for each club, or fill it in online, where it saves in your own browser, and print from there.

Formats

  • Printable PDF (A4 and US Letter)
  • Fillable PDF with form fields
  • Fill in online, saved in your browser

The printable is laid out inside a box that fits both A4 and US Letter, so the two files are identical and neither one is scaled. The fillable PDF has real form fields. The online version keeps what you enter in your own browser and builds the completed PDF on your device.

How to print

Download the A4 or US Letter file to match the paper in your printer, open it in a PDF reader and print at 100% — not "fit to page", which shrinks the writing space. Both files leave a 14 mm margin, which clears the unprintable edge on home printers. To print the online version with your entries in it, use the Print this sheet button, which prints only the sheet and none of the page around it. More on printing.

Questions

Why write the dose exactly as prescribed?

Because the leader will follow what is on the form. Copying the label word for word avoids a second version of the instructions. This sheet never suggests a dose; it only records the one you have been given.

Can I give consent for some activities and not others?

Yes. Each consent is a separate tick, and an unticked line means no. Add your own lines in the browser version or on the blank lines on paper for anything specific to the club.

Does the club have to accept this form?

Some clubs and schemes insist on their own paperwork, and the rules on medicines and consent differ by country and organisation. Check with the club; many are happy to take this alongside theirs.

What if the details change mid-term?

Fill in a new form, date it and hand it over, and ask the leader to replace the old one. Keeping one current form is safer than crossing out on an old copy.
