What this sheet is for

Keep observations and conclusions in separate columns, which is what makes a record useful when somebody else reads it back.

Who it is for

Supervisors, surveyors, wardens, researchers, wildlife recorders, care staff and anyone whose notes may be read by somebody official.

How to use it

  1. Write what you actually saw or heard, in plain words.
  2. Keep your interpretation in its own column.
  3. Record date, time and place for every entry.
  4. Note who you passed it to, and when.
  5. Sign it. An unsigned observation is an anecdote.

What is on the sheet

The layout is fixed at a size that fits both A4 and US Letter, so the A4 and Letter files are visually identical and print at full size on either. Rows are set at a height that an adult can actually write a date and a few words into with a pen — most printable tables are built in a word processor and never tested with one.
