What this sheet is for

Record a day as it happens in enough detail that it can be reconstructed later by somebody who was not there.

Who it is for

Shift workers, caretakers, security, care and support staff, site supervisors, and anyone keeping a day book.

How to use it

  1. Write the times in yourself — days start at different hours.
  2. Record entries as they happen, not at the end of the shift.
  3. Note who was involved, by name.
  4. Carry anything unfinished forward to the next sheet explicitly.
  5. Sign the bottom of each sheet.

What is on the sheet

The layout is fixed at a size that fits both A4 and US Letter, so the A4 and Letter files are visually identical and print at full size on either. Rows are set at a height that an adult can actually write a date and a few words into with a pen — most printable tables are built in a word processor and never tested with one.
