What this sheet is for
Record a day as it happens in enough detail that it can be reconstructed later by somebody who was not there.
Who it is for
Shift workers, caretakers, security, care and support staff, site supervisors, and anyone keeping a day book.
How to use it
- Write the times in yourself — days start at different hours.
- Record entries as they happen, not at the end of the shift.
- Note who was involved, by name.
- Carry anything unfinished forward to the next sheet explicitly.
- Sign the bottom of each sheet.
What is on the sheet
One page, top to bottom:
Printed under the title: A day, hour by hour, with room to write what happened in each one.
Write your own times down the side. For anything that has to be reconstructed afterwards — a shift, a callout, a day of a job, a day of an illness. The times down the side are yours to write, because days start at different hours.
