What this sheet is for

Turn a note into a record, by giving it a title, a date, ruled space and somewhere to sign.

Who it is for

Anyone who needs to write something down properly and keep it: statements, handovers, meeting notes, findings, day books.

How to use it

  1. Fill in the title block first. An untitled sheet is worthless in a year.
  2. Write in ink if it may be read back by somebody else later.
  3. Sign and date it, even when it is only for you.
  4. Number the pages if it runs to more than one.
  5. File it where the related paperwork already lives.

What is on the sheet

The layout is fixed at a size that fits both A4 and US Letter, so the A4 and Letter files are visually identical and print at full size on either. Rows are set at a height that an adult can actually write a date and a few words into with a pen — most printable tables are built in a word processor and never tested with one.
