Who this is for

A ruled log with the lines already numbered and nothing else decided. Write what the log is for in the box at the top, then take one numbered line per entry: the date, the time, what happened, a reference and whose initials.

Anyone who needs a log for something no printed form covers — a plant room, a reception desk, a workshop bench, a committee, a boat, a radio room or a one-off job that has to leave a trail.

What this helps you track

  • A numbered line per entry, so a missing entry is obvious
  • Date, time and initials on every line without having to rule them yourself
  • A wide column for the entry itself, rather than a cramped box
  • A reference column to tie the line to a docket, an invoice or a photograph
  • A title box, so one sheet serves for any log you need

How to use it

  1. Write what the log records in the title box, with the site and who keeps it.
  2. Number the sheet itself and file the sheets in order — the line numbers only help if the pages stay in sequence.
  3. Take the next numbered line for each entry and write the date and time as you go, not afterwards.
  4. Keep entries factual and short: what happened, where and who was involved.
  5. Put a docket, job or photograph reference in the last column so the entry can be followed up.
  6. Initial every line, and rule a line through anything left blank at the end of the sheet.

Formats

  • Printable PDF (A4 and US Letter)
  • Fillable PDF with form fields
  • Fill in online, saved in your browser

The printable is laid out inside a box that fits both A4 and US Letter, so the two files are identical and neither one is scaled. The fillable PDF has real form fields. The online version keeps what you enter in your own browser and builds the completed PDF on your device.

How to print

Download the A4 or US Letter file to match the paper in your printer, open it in a PDF reader and print at 100% — not "fit to page", which shrinks the writing space. Both files leave a 14 mm margin, which clears the unprintable edge on home printers. To print the online version with your entries in it, use the Print this sheet button, which prints only the sheet and none of the page around it. More on printing.

Questions

Why numbered lines?

Because a numbered log makes a gap visible. If line 14 is missing, somebody can ask about it; on a plain ruled sheet nobody can tell.

How is this different from the blank log sheet?

That one is an unnumbered ruled grid. This has numbered lines, a time column, a reference column and a title box, which makes it closer to a bound logbook page than a sheet of lines.

Should I cross out the unused lines?

Many record-keeping conventions say so, and it costs nothing. A struck-through line is one nobody can fill in later.

Can I use it as a continuation sheet for another form?

Yes — that is one of the better uses for it. Write the form's name and reference in the title box and carry on from where the other sheet ran out.
