Who this is for

The job sheet for work done on somebody's driveway. It identifies the vehicle by registration and mileage, separates the reported fault from the one you found, lists the parts actually fitted and totals the labour at your own rate.

Mobile mechanics, roadside repairers and small independent garages that work away from a ramp and need a signed record before they drive off.

What this helps you track

  • The vehicle by registration, make, model and the mileage on arrival
  • The fault reported, the fault found and the work done, in that order
  • Parts fitted with part numbers, and who supplied them
  • Labour hours priced at your own rate, plus any call-out you charge
  • A signature at the roadside against a vehicle handed back

How to use it

  1. Take the registration, make, model and the odometer reading before you start, and note where the vehicle is.
  2. Write the fault as the customer described it, then what you found once you had looked.
  3. Enter the labour: each spell of work, the hours as a decimal and your rate. A call-out charge goes on its own line.
  4. List the parts fitted with part numbers and quantities, and tick whether the customer supplied them — those are usually not your warranty.
  5. Run the post-work checks, read the totals, and have the customer sign for the vehicle before you leave.

Formats

  • Printable PDF (A4 and US Letter)
  • Fillable PDF with form fields
  • Fill in online, saved in your browser
  • Excel / Google Sheets workbook

The printable is laid out inside a box that fits both A4 and US Letter, so the two files are identical and neither one is scaled. The fillable PDF has real form fields. The workbook uses plain formulas only — SUM, IF, COUNTIF and the like — so it opens in Excel, LibreOffice and Google Sheets without anything breaking, and every calculated cell is grey so you can see which figures it worked out.

How to print

Download the A4 or US Letter file to match the paper in your printer, open it in a PDF reader and print at 100% — not "fit to page", which shrinks the writing space. Both files leave a 14 mm margin, which clears the unprintable edge on home printers. To print the online version with your entries in it, use the Print this sheet button, which prints only the sheet and none of the page around it. More on printing.

Questions

What if the customer supplies the parts?

Tick the customer column on that line and leave the price blank or at zero. It is worth recording, because a part you did not supply is usually outside whatever guarantee you give on your own work.

Should I record the mileage?

Yes, on arrival at least. It dates the work against the vehicle's history, settles arguments about what was done when, and belongs on any invoice that follows the vehicle to its next owner.

Does this cover a road test?

There is a check for it, and a line in the notes for what you found. Write the distance and anything you noticed; an unexplained noise on a road test is worth a sentence on the sheet.

Can I use it for a service rather than a repair?

Yes — list the service items as labour lines and the filters, oil and plugs as parts. The vehicle service record on this site is the better home for the customer's own long-term history.
