The same sheet, on screen. Totals and figures are worked out as you type, rows can be added or removed, and you can print it or download the completed version as a PDF. Everything stays in your browser — your entries are kept in this browser's own storage on this device, nothing is uploaded, and there is no account.

Who this is for

One appliance, one sheet. It pins down the make, model and serial number, records whether the repair is chargeable or covered by a warranty or contract, and totals the labour and the parts you fitted at your own rate.

White goods and domestic appliance engineers, in-warranty repairers and small repair shops collecting or visiting, who bill some jobs and claim others.

What this helps you track

  • The appliance identified by make, model and serial number
  • Whether the visit is chargeable, under warranty or on a contract
  • The fault reported, the fault found and the test that proved the repair
  • Parts fitted or ordered, with part numbers and cost
  • Labour hours at your own rate, and the total for the job

How to use it

  1. Copy the make, model and serial number off the rating plate — not off the customer's memory — and note where the appliance is installed.
  2. Record the warranty or contract reference if there is one, and whether proof of purchase was seen.
  3. Write the fault reported, then the fault found, then how you proved the repair worked.
  4. Enter the labour hours against your rate, and the parts with part numbers. Tick anything on order for a return visit.
  5. Check the totals, tick the closing checks and have the customer sign that the appliance was left working.

Formats

  • Printable PDF (A4 and US Letter)
  • Fillable PDF with form fields
  • Fill in online, saved in your browser
  • Excel / Google Sheets workbook

The printable is laid out inside a box that fits both A4 and US Letter, so the two files are identical and neither one is scaled. The fillable PDF has real form fields. The workbook uses plain formulas only — SUM, IF, COUNTIF and the like — so it opens in Excel, LibreOffice and Google Sheets without anything breaking, and every calculated cell is grey so you can see which figures it worked out.

How to print

Download the A4 or US Letter file to match the paper in your printer, open it in a PDF reader and print at 100% — not "fit to page", which shrinks the writing space. Both files leave a 14 mm margin, which clears the unprintable edge on home printers. To print the online version with your entries in it, use the Print this sheet button, which prints only the sheet and none of the page around it. More on printing.

Questions

How do I handle an in-warranty job?

Fill it in exactly the same way and note the warranty or contract reference at the top. Leave the rate blank if you are not billing the customer — the sheet still records the hours and parts you will claim.

The serial number is unreadable. Does it matter?

It usually does, because manufacturers match parts and warranty claims to it. Write where you looked and what you could read; photograph the plate if the numbers are faint.

What if the appliance is not worth repairing?

Record the fault found and your assessment in the notes, charge for the visit as a labour line, and have the sheet signed. It becomes the customer's evidence for an insurance or warranty claim.

Can I use it for commercial catering equipment?

Yes — the make, model, serial and warranty structure is the same. Add the site and location fields for equipment that sits in a kitchen with several similar machines.
