A free printable ESD test log: each person's daily wrist strap and footwear result with the reading, the action on a fail, and weekly bench grounding checks.

The fillable versions have real form fields — open them in any PDF reader, type into them and save. Printing help.
Show that everyone at the bench tested their wrist strap and footwear before starting work. A failed strap only does harm if nobody notices and someone works on with it.
Electronics repair benches, assembly lines, labs and anyone running an ESD-protected area with a wrist strap and footwear tester.
2 pages, in this order:
Printed under the title: One line per person per test at an ESD-protected bench: the wrist strap and each shoe or heel grounder, pass or fail with the reading, and what was done about a fail. The bench's own weekly checks are at the end.
Your programme sets the pass band. No resistance limits are printed. The pass band for wrist straps and for footwear comes from your own static control programme and the tester's settings: write both in the boxes below, with where they come from. Anyone whose strap or footwear fails stays off the bench until a retest passes.

Because the acceptable band depends on your static control programme, the standard it follows and how your tester is set. A printed range could be the wrong one for your bench. Write yours at the top, with where it comes from, and judge every test against it.
Some programmes allow continuous monitors in place of a daily test and others do not. Follow your own programme; if you use monitors, this sheet still suits the footwear tests and the weekly bench checks.

Record what protective equipment was issued to whom and when it was replaced, with the instruction and fit-check noted at the point of issue.
Keep a per-machine service history for fixed workshop equipment, so the consumable part numbers and the last service date live with the machine.
