What this sheet is for

Record what protective equipment was issued to whom and when it was replaced, with the instruction and fit-check noted at the point of issue.

Who it is for

Small employers, site managers, farms, workshops, kitchens and cleaning contractors.

How to use it

  1. Record the standard marked on the item, not just what it is.
  2. Note that instruction and a fit check were given — especially for respirators.
  3. Get a signature on receipt.
  4. Record replacements on the same sheet so the pattern of wear is visible.
  5. Note anything reported as uncomfortable or unsuitable; it will not be worn otherwise.

What is on the sheet

The layout is fixed at a size that fits both A4 and US Letter, so the A4 and Letter files are visually identical and print at full size on either. Rows are set at a height that an adult can actually write a date and a few words into with a pen — most printable tables are built in a word processor and never tested with one.

No. Selection comes from your own risk assessment, the manufacturer's information and local rules. This records issue and replacement only.
