What this sheet is for

Send the league a card record both captains and the umpires agreed at the pitch. Nothing is reconstructed from memory on Monday.

Who it is for

Umpires, captains and match secretaries at club hockey, from junior sides upwards.

How to use it

  1. Fill in both teams, the officials and the push-back time before the start.
  2. Enter each goal as it is scored — time, quarter, scorer and whether it came from open play, a penalty corner or a stroke.
  3. Write every card on its own line with the time it was shown and when the player went off.
  4. Agree the result and have both captains and the umpires sign.
  5. Report the cards wherever your league asks for them.

What is on the sheet

One page, top to bottom:

Printed under the title: The match record as the umpires and captains keep it — every goal and every card in the order they happened, with the time, the player and when a carded player went off and came back. How long a card keeps a player off, and what gets reported, are your governing body's rules: the columns record what happened, not what the rule is.

Write-on lines: Home team · Away team · Competition · Date · Venue / pitch · Push back · Umpire 1 · Umpire 2 · Technical official

Goals and cards — one line per event, in order — goals and cards together

Table, 12 rows: Time · Qtr · Team · Player (no. / name) · Goal: how · Card · Reason · Off at · Back on · Score

Write-on lines: Half-time · Full-time · Shoot-out · Cards reported to league by · On

Signature strip: Home captain · Away captain · Umpires

Questions

Why are the time-off columns blank rather than printed with the suspension length?

Because how long a green or yellow card keeps a player off, and what happens after a second card, are set by your governing body and can differ between competitions. The sheet records the times that actually happened.
