Match a second sitting or a touch-up to the first, because the needle configuration, the ink brand and colour and the lot number are all written down against the date.
Tattoo artists, whether in a studio or working guest spots, who want one card per piece rather than a diary entry.
One page, top to bottom:
Printed under the title: One card per tattoo — the design as agreed at consultation, a space for the placement sketch, then every session logged with the needles and inks used, so a touch-up or a second sitting matches the first.
Your policy fills the blanks. Deposit amount, minimum age, ID and consent wording all come from your own studio policy and the rules where you work. The blanks below are for your figures and your checks; nothing here says what they have to be.
Write-on lines: Client · Contact · Artist · Consultation date · ID / age checked (per studio policy) · Consent form signed · Deposit taken · Medical / allergy notes on file
Write-on lines: Description / subject · Style · Colour or black & grey · Placement · Approx. size · Reference / stencil ref. · Cover-up or existing work
Squared grid: Placement sketch
Table, 4 rows: Date · Session · Work done · Needles / cartridges · Inks (brand, colour, lot) · Time · Healed check · Artist
Because the age you can tattoo, the ID you accept and the consent wording are set by the rules where you work and by your own studio, not by us. The blanks record that your checks were done.
On the Tattoo Aftercare Handout Record, which logs which version of your handout the client was given. Keeping it separate keeps this card about the work itself.
Prove, months later, that a client left with the aftercare sheet in their hand and had it talked through, and know exactly which wording they were given.
Show that every chair, bed or booth was reset between people, and that the studio was properly closed down each night, without relying on anyone's memory of a busy Saturday.
Keep one card per customer so the history of what was done, what it cost and when the next visit is due lives in one place.
Keep one signed record of what each person actually agreed to, so a picture is not used somewhere nobody discussed.
Every sheet, binder, game and puzzle on Upkeep Papers is free to download, with no email and no account. Making them is not free: each one takes days of research, design and checking, and the site costs money to run. If something here saved you time, you can buy us a coffee. It is entirely optional: nothing is locked, and nothing changes if you don’t.
