Photo Shoot Planner and Shot List
Write the shot list before the shoot rather than in the car afterwards, and tick shots as captured so nothing is assumed.

A free printable photo release and consent record — who was photographed, when and where, what use they agreed to, who signed and how to reach them.
One page. Choose your paper size — both versions are laid out identically, so neither one gets shrunk to fit.
Print it and write on it
Fill it in type and save
The fillable versions have real form fields — open them in any PDF reader, type into them and save. Printing help.
Keep one signed record of what each person actually agreed to, so a picture is not used somewhere nobody discussed.
Photographers and videographers working with people, and anyone shooting for a club, school, charity or employer.
One page, top to bottom:
Printed under the title: A record of who agreed to be photographed or filmed, when, where, and what use they agreed to — one signed row per person, kept with the shoot.
Check the rules where you are. What a release needs to say, who can sign for a child, and how long you keep it all depend on where you are working and who the pictures are for — check the rules where you are, and use your own wording in the 'use agreed' column. This sheet is the record that the conversation happened and what was agreed, not a legal form.
Write-on lines: Photographer / business · Shoot or job reference · Date of shoot
Table, 9 rows: Name · Date · Location · Use agreed · Signed by (if not the subject) · Contact · Signature
No. It is a record of what was agreed and who agreed to it. What a release has to say, who may sign on someone else's behalf and how long you keep it all vary by where you are and who the pictures are for — check the rules where you are, and take advice if the work needs it.
Because the agreed use is different every time — a portfolio, a client's website, a print in an exhibition, a press release. Printing one form of words would only get signed without being read. Write what was actually agreed.
Write the shot list before the shoot rather than in the car afterwards, and tick shots as captured so nothing is assumed.
Get through the day without missing the shots that cannot be taken again. The family groups run quickly because the names are already written down.
Arrive on the shoot day knowing where to park, where the power is and that someone in authority has actually said yes.
Know who came, and have something to work from afterwards, without collecting more about people than the event actually needs.
The same subject, with a version you can fill in on screen.
The episode number, title and both the recording and release dates.
Every sheet, binder, game and puzzle on Upkeep Papers is free to download, with no email and no account. Making them is not free: each one takes days of research, design and checking, and the site costs money to run. If something here saved you time, you can buy us a coffee. It is entirely optional: nothing is locked, and nothing changes if you don’t.
