What this sheet is for

Have the product, lot number and skin reaction from last time in front of you before you start this time, on one card per client.

Who it is for

Beauty therapists, waxing and skincare specialists, and salons that need a per-client record their insurer will accept.

How to use it

  1. Open one card per client and fill in the declared conditions and allergies before the first treatment.
  2. Record the patch test product, date and result on the line provided.
  3. Log every treatment with the product and its lot or batch number, not just the brand.
  4. Note any reaction, even a mild one, and read it back before the next treatment.
  5. Write in the next visit date if one was booked.

What is on the sheet

One page, top to bottom:

Printed under the title: A running record for one client across every beauty treatment — what was done, which product and lot number, who did it and how the skin reacted.

Your training and your insurer decide. When a patch test is needed, how far ahead, and what counts as a contra-indication come from your training, your insurer, the product manufacturer and the rules where you work. This card records the answers; it does not supply them.

Write-on lines: Client · Contact no. · Date of birth · Therapist · Medical conditions, medication or allergies declared · Declared on · Patch test — product · Date done · Result · Consent form signed on

Questions

Why does the card not say when a patch test is needed?

Because that depends on the product, its manufacturer, your insurer and the rules where you work, and they do not agree with each other. The card records that a test was done, on what and with what result; when to do it is your training's call.

Why record the lot number and not just the product?

If a client reacts, the lot number is what the manufacturer and your insurer will ask for. It is on the tube or bottle at the moment you use it and hard to find afterwards, so write it down then.
