Salon Client Consultation Card

A free printable salon consultation card — what the client wants, their hair history, allergies and patch test, your assessment, the agreed plan and both signatures.

First page of the Salon Client Consultation Card — Ask every question before the first cut or colour, write the answers down, and have the client sign what was agreed.

What this sheet is for

Ask every question before the first cut or colour, write the answers down, and have the client sign what was agreed.

Who it is for

Hairdressers and colourists taking on a new client, and salons that want the same consultation done at every chair.

How to use it

  1. Fill in the client's details and how they found you.
  2. Write what they want in their own words, and what they do not want.
  3. Go through the hair history line by line — previous colour, box dye, heat, extensions — and the allergies and patch test lines.
  4. Record your own assessment and the service you agreed, with your quote.
  5. Both sign, then keep the card with the client's colour record.

What is on the sheet

One page, top to bottom:

Printed under the title: The first conversation, written down — what they want, what their hair has been through, and what you both agreed and signed.

It records the answers, not the rules. What a head of hair can take after a box dye, a relaxer or a bleach is a judgement from your own training and the manufacturer's guidance, not from this card. It makes sure you asked, and records what the client told you and what you agreed.

Write-on lines: Client · Contact no. · Date · Stylist · How they heard of you · Reference photo brought? · Previous salon / stylist

What they want

Write-on lines: In their words — the look, the length, the colour · What they do not want · Time they spend on it at home

Hair history — ask, then write down what they told you

Write-on lines: Colour or chemical services in the last year (what, when, where) · Home colour or box dye? · Heat styling · Products used at home · Extensions / pieces · Allergies, sensitivities or reactions declared · Patch test date · Result · Scalp condition

Your assessment and the plan

Write-on lines: Texture · Density · Porosity · Elasticity · Natural base · Grey % · Agreed service, technique and expected result · Quoted (your price) · Sittings · Home care and aftercare discussed (your own advice)

Signature strip: Client signature · Stylist signature · Date

Questions

Why is there no guidance on what to do when the history is bad?

Because whether hair with a year of box dye or a relaxer in it can take the service is a judgement from your training and the manufacturer's guidance, not from a printed card. The card makes sure the question was asked and the answer written down.

Do I need a separate colour record too?

This card is the first conversation. The Colour Formula and Development Record is the running card for every colour visit after it — formula, developer, time and result. Keep the two together.
