Salon Client Consultation Card
Ask every question before the first cut or colour, write the answers down, and have the client sign what was agreed.

A free printable hair colour record card — natural base, hair history and patch test, then every colour service with formula, developer, time and result.
One page. Choose your paper size — both versions are laid out identically, so neither one gets shrunk to fit.
Print it and write on it
Fill it in type and save
The fillable versions have real form fields — open them in any PDF reader, type into them and save. Printing help.
Match last time's colour exactly, or change it on purpose, because the formula and the time it was left are written down rather than remembered.
Colourists and hairdressers, and salons where a client's colour may be done by more than one person.
One page, top to bottom:
Printed under the title: One card per colour client — the exact formula mixed, the developer, the technique and the time it was left, every visit, so the next one can match the last or move on from it.
Your formula, the manufacturer's chart. The formulas, developer strengths and development times on this card are the ones you mixed and used, from your own training and the manufacturer's shade chart. None are printed here. The patch test line records that one was done and what it showed — when it is needed is set by the manufacturer and the rules where you work.
Write-on lines: Client · Contact no. · Stylist · Card no. · Natural base and % white · Hair history (previous colour, bleach, relaxer, henna) · Porosity / condition · Patch test — product · Date · Result · Allergies or reactions declared
Table, 9 rows: Date · Service · Formula (brand, shade, ratio) · Developer · Technique · Time · Result · By
Because the formulas belong to the manufacturer's shade chart and the timings to your training and their instructions. The card records what you chose and what it did on this head of hair, which is the only thing that helps next time.
There is a line for the product tested, the date and the result. How far ahead of a service it has to be done, and how often it has to be repeated, is set by the manufacturer and the rules where you work — not by this card.
Ask every question before the first cut or colour, write the answers down, and have the client sign what was agreed.
Order the shades you actually go through instead of the ones the rep suggests, and never find the shelf empty mid-application.
Have the product, lot number and skin reaction from last time in front of you before you start this time, on one card per client.
Have the adhesive, tint shade, curl and timing from the last set in front of you before you start the next, with the patch test and reaction on the same page.
Every sheet, binder, game and puzzle on Upkeep Papers is free to download, with no email and no account. Making them is not free: each one takes days of research, design and checking, and the site costs money to run. If something here saved you time, you can buy us a coffee. It is entirely optional: nothing is locked, and nothing changes if you don’t.
