What this sheet is for

Reorder the exact piece that went in, and see the history of a site when a client comes back with a concern or for a downsize.

Who it is for

Body piercers in a studio or a salon with a piercing chair, keeping a card per client rather than a single running log.

How to use it

  1. Fill in the client's details, that ID was checked, and that consent was signed.
  2. For each piercing, record the site and the jewellery as your supplier specifies it, plus the needle and its lot.
  3. Log every return visit — check, downsize, concern — with how the site looked and any jewellery change.
  4. Read the card before the client sits down again.

What is on the sheet

One page, top to bottom:

Printed under the title: One piercing client's card — each piercing done, the jewellery put in, the needle used, and what happened at any check or downsize afterwards.

Supplier's spec, your record. Jewellery material, gauge and length are written as your supplier states them, so the same piece can be reordered. What you tell a client about healing is yours and the rules where you work; this card only records what went in and when it was changed.

Questions

Why is there no aftercare or healing time on the card?

Because what you tell a client and how long a site takes come from your training and the rules where you work, and they vary by site and by person. The card records what went in and what happened at each visit.

Why record the needle lot?

So that if a supplier recalls a batch, or a client has a problem, you can say exactly which needle was used on which date.
