What this sheet is for

Have the make, model and serial to hand when something breaks, and a history that shows which items keep failing and which are due a check.

Who it is for

Salon, spa and studio owners with equipment that gets tested, serviced or repaired by someone else and want one place to record it.

How to use it

  1. List each item once from its rating plate or label, with where it lives.
  2. Write in the electrical test date your tester gave you, and the next service date from the manufacturer's guidance.
  3. Log every fault, service and repair with who did it and what it cost.
  4. Update the item row when something is replaced.

What is on the sheet

One page, top to bottom:

Printed under the title: Chairs, dryers, couches, wax pots, steamers and sterilisers as equipment with a history — identified once, then every fault, service and repair against the item it belongs to.

The manufacturer's interval, your log. How often a piece of equipment is checked or serviced is set by its manufacturer, your insurer and the rules where you work — write that date in the 'next due' column from their guidance. This sheet is the equipment's own history, not a schedule.

Questions

Why are there no service intervals or test frequencies printed?

Because they come from each item's manufacturer, your insurer and the rules where you work, and they differ from a hairdryer to an autoclave. The columns take their dates; the sheet does not set them.

Does the steriliser go here or on the Sterilisation Cycle Log?

Both. Its identity and its repairs live here; its individual runs and tests live on the cycle log.
