What this sheet is for

Show, for any container, who put it into use, who sealed it and when it left the premises with whom — without hunting through a drawer of receipts.

Who it is for

Tattoo studios, piercers, and salons or clinics that use needles, lancets or blades and have a waste carrier collect them.

How to use it

  1. Number each container as it is assembled, and log the date and who did it.
  2. When a container is sealed, log the date, who sealed it and that it was locked and labelled.
  3. At each collection, record which container numbers went, the carrier and the consignment note number.
  4. Keep the consignment notes with this sheet.

What is on the sheet

One page, top to bottom:

Printed under the title: Each sharps container from assembly to collection — who put it into use, who sealed it, and the carrier's paperwork when it left the premises.

Your carrier's rules, your record. How full a container may be before it is sealed, who may collect it and what paperwork must go with it are set by the rules where you work and your waste carrier. This sheet records that each step happened and who did it.

Questions

Why is there no fill line or time limit on the sheet?

Because how full a container may be, how long it may stay in use and what paperwork must travel with it are set by the rules where you work and your waste carrier's contract. The sheet records that each step was done.

What if a container is sealed but not yet collected?

It stays on the containers table with its sealed date, and appears on the collections table when it goes. The two dates together show how long sealed waste sat waiting.
