Tattoo studios, piercers, beauty salons, podiatry and clinic rooms running an autoclave or steriliser for reusable tools.
One page, top to bottom:
Printed under the title: Every run of an autoclave or steriliser — what was in it, who ran it, what the display and the indicator showed, and whether the load was released.
Their figures, your readings. The values a cycle has to reach, the indicators you use and how often the machine is tested are set by its manufacturer and the rules where you work. Write what the display and the indicator actually showed; whether that passes is a judgement you make against their figures, not ours.
Write-on lines: Machine make & model · Serial no. · Location · Month
Table, 6 rows: Date · Cycle no. · Load contents · Programme · Time started · Display reading · Indicator · Released · Operator
Table, 4 rows: Date · Test or event · Result · Action taken · Initials
Because the values a cycle must reach are set by the machine's manufacturer and the rules where you work, and differ by machine and programme. The column takes what the display showed; whether that passes is judged against their figures.
Mark the indicator or release column as failed, note what you did in the tests and faults table, and do not release the load. What happens next is your procedure and the manufacturer's.

Have the make, model and serial to hand when something breaks, and a history that shows which items keep failing and which are due a check.
Reorder the exact piece that went in, and see the history of a site when a client comes back with a concern or for a downsize.
Show that an appliance was checked, twice a day, by someone who wrote down what they saw. A run of readings shows drift long before one reading shows a failure.

Making them is not free: each one takes days of research, design and checking, and the site costs money to run. If something here saved you time, you can buy us a coffee.
