Massage and Body Treatment Notes

A free printable massage client record sheet — one card per client with treatment, areas worked, pressure, product used and client feedback, one row per visit.

First page of the Massage and Body Treatment Notes — Start each session from what actually happened last time — the knot that was found, the pressure they asked for, the oil they liked — instead of asking again.

What this sheet is for

Start each session from what actually happened last time — the knot that was found, the pressure they asked for, the oil they liked — instead of asking again.

Who it is for

Massage therapists, body treatment therapists and spa staff keeping a card per client, whether in a salon, a clinic room or on the road.

How to use it

  1. Fill in the client's details once, and note when the consultation form and consent were completed.
  2. After each visit, add a row: treatment, areas worked, pressure and the product as it is named on the bottle.
  3. Write what the client said during and after, in their words.
  4. Read the last row before the client comes in next time.

What is on the sheet

One page, top to bottom:

Printed under the title: One client's card for massage and body treatments — what was done, where, how firm, with what, and what they said about it, so the next session starts from the last one.

Your consultation, your call. This card records what you did and what the client told you. Which conditions rule a treatment out, what you ask before you start and what you tell a client afterwards come from your own training, your insurer and the rules where you work — not from this sheet.

Questions

Why is there nothing about contraindications on the card?

Because which conditions rule a treatment out, and what you ask about, come from your training and your insurer, and differ by treatment and country. The card has a line to note that the consultation was done and what was raised, not a list of what to ask.

Where do the consultation form and consent go?

Keep them with this card. The card notes the date they were completed so you can see at a glance whether they need refreshing under your own rules.
