What this sheet is for

Record the check before driving and the journeys after it on one sheet per day.

Who it is for

Van drivers, fleet operators, minibus and community transport, delivery drivers and anyone whose vehicle is someone else's responsibility.

How to use it

1. Do the walk-round before the first journey, not after it.
2. Write your own check items in — they depend on the vehicle.
3. Record every defect, even the ones you are going to live with, and who you told.
4. Log journeys with odometer readings.
5. Sign it and hand it in.

What is on the sheet

The layout is fixed at a size that fits both A4 and US Letter, so the A4 and Letter files are visually identical and print at full size on either. Rows are set at a height that an adult can actually write a date and a few words into with a pen — most printable tables are built in a word processor and never tested with one.

What has to be checked?

That depends on the vehicle, its weight, its use and the country, so the check items are deliberately blank. Write in the list that applies to you and use the same one every day.

Is a driver defect report a legal requirement?

For some vehicles and operations, in many countries, yes — and the rules are specific. Establish what applies to you; this sheet gives you somewhere to record it.
