A kitten's first twelve months written down as they happen. Vet visits and treatments go in the first table with the next date the practice gave you, weighings in the second, and the third is for handling, new rooms, carriers, other animals and everything else a kitten has to get used to.
New kitten owners, fosterers raising a litter to homing age, and anyone who wants the record to go to the next owner rather than staying on a practice computer.
The printable is laid out inside a box that fits both A4 and US Letter, so the two files are identical and neither one is scaled. The fillable PDF has real form fields. The online version keeps what you enter in your own browser and builds the completed PDF on your device.
Download the A4 or US Letter file to match the paper in your printer, open it in a PDF reader and print at 100% — not "fit to page", which shrinks the writing space. Both files leave a 14 mm margin, which clears the unprintable edge on home printers. To print the online version with your entries in it, use the Print this sheet button, which prints only the sheet and none of the page around it. More on printing.
No. There is no schedule anywhere on the sheet. Each due date is one your vet gave you, written in the column for it, because timings differ by vaccine, by country and by cat.
This is a wider first-year record covering visits, treatments, weight and settling in. The vaccination record is the ongoing list of what was given and when the next is due, and it suits an adult cat.
Because a cat that has been handled, carried and met other animals calmly is easier to treat and to rehome. Dated lines also show a vet or a new owner what the kitten has actually experienced.
Write the date it arrived and the estimated age your vet gave you, and mark the rest unknown. The record starts where your knowledge does.
Move the entries onto the cat vaccination record and the vet visit record and keep this sheet with them as the early history.

Weight at each weighing, with the date and the scales used.
The chip number written somewhere other than a database you cannot log into.
Every appointment in date order with the reason you went.
Printable sheets, workbooks and tools on the same subject.
Hold a cat's identification, insurance and treatment history on one page that a cattery, a sitter or a new practice can read. It has room for the things that are specific to cats and get left off general pet forms.
Track weight against what the animal is actually being fed, so a change in one can be read against the other. A single weighing tells you very little; a column of them taken the same way each time is something a vet can use.
Every sheet, binder, game and puzzle on Upkeep Papers is free to download, with no email and no account. Making them is not free: each one takes days of research, design and checking, and the site costs money to run. If something here saved you time, you can buy us a coffee. It is entirely optional: nothing is locked, and nothing changes if you don’t.
