Wheel Alignment Record
Keep the alignment result after the rig's printout has faded, with the manufacturer's figures written beside your own so the sheet stands alone.
Garage and vehicle workshop · GV-36
A free printable ADAS calibration record: which driver assistance sensors the car has, which the repair affected, and how each was calibrated, by whom, with result.
One page. Choose your paper size — both versions are laid out identically, so neither one gets shrunk to fit.
Print it and write on it
Fill it in type and save
The fillable versions have real form fields — open them in any PDF reader, type into them and save. Printing help.
Show which driver assistance sensors were checked, calibrated and proved after the repair. The sheet stays with the job, with the equipment printout attached.

They differ for every make, model and sensor, and they come from the vehicle maker's repair information and your calibration equipment. The sheet records what was done against that information, not the procedure itself.
Tick that it could not be determined, write what you tried, and tell the customer. Recording the doubt is better than leaving the box blank.
Keep the alignment result after the rig's printout has faded, with the manufacturer's figures written beside your own so the sheet stands alone.
Separate what the car arrived with from what was claimed for, and keep the actual hours and materials against each stage so the invoice and the estimate can be compared honestly.
Turn a diagnostic session into a record the next technician can pick up. It also gives the customer something better than 'it had a code'.
Turn 'took it round the block, seemed fine' into a record of what was actually tried, on what road, by whom — before the car goes back to its owner.
