One page, top to bottom:
Printed under the title: Stock going back to a supplier — damaged, faulty, wrong or over-delivered — with the reason, the supplier's reference, when it left and whether the credit ever arrived.
Each supplier's terms, not ours. Whether a line can go back, how long you have to report it and whether you get a credit note or a replacement is set by each supplier's own returns terms. This sheet does not know those terms; it records what was claimed and closes the loop when the credit shows up.

Trace a fixture change, a stock movement or a missing line back to the trade visit that caused it, instead of finding it at stocktake with no explanation.
Keep the ordering knowledge out of one person's head and on a page anybody could pick up.
Count stock area by area and see the variance beside the count, so a stock take produces a number you can act on rather than a vague reassurance.
