Poultry Parasite and Treatment Record

What this sheet is for

Record treatments with the two withdrawal dates that matter, so eggs and meat are not used before they are clear.

Who it is for

Backyard keepers, smallholders and anyone selling or eating their own eggs.

How to use it

1. Copy the product name and batch exactly from the container.
2. Read both withdrawal periods — egg and meat are usually different.
3. Work out the dates each one ends and write them down.
4. Record who was treated well enough to identify them again.

What is on the sheet

The layout is fixed at a size that fits both A4 and US Letter, so the A4 and Letter files are visually identical and print at full size on either. Rows are set at a height that an adult can actually write a date and a few words into with a pen — most printable tables are built in a word processor and never tested with one.

Questions

Why does the sheet not say what to treat with?

Because it depends on the parasite, the country and often a vet's prescription, and many products sold for poultry are not licensed for laying birds. Getting this wrong puts contaminated eggs on a table. Your vet and the label are the sources.

Is a record legally required?

In many countries, yes, for food-producing animals — and backyard hens usually count as food-producing even when the eggs never leave the garden. Check locally.
