A free printable product recall form — the affected lots, the reason, who was notified, each customer traced with quantities returned or destroyed.

What this sheet is for

Handle a bad batch in order and prove afterwards exactly what was pulled, from where and when, rather than reconstructing it from phone calls.

Who it is for

Any small food producer who supplies shops, markets, caterers or the public — which is everyone the rest of this category is written for.

How to use it

  1. The moment a problem is found, write in the product, the lot and date codes, and the reason.
  2. Check the rules where you trade and any scheme you belong to; note who you told and what they asked of you.
  3. From your batch and sales records, list every place the affected lots went.
  4. Contact each one, then record what came back and what was destroyed.
  5. Close it out with the cause and the change made, and sign it.

What is on the sheet

One page, top to bottom:

Printed under the title: One product problem from the moment it is decided to pull stock — what is affected, why, who was told and when, and what came back or was destroyed — so the whole episode is on one sheet.

Who has to be told is not printed here. Whether a problem is a withdrawal from your own shelves or a recall from customers, and who has to be notified, is set by the rules where you trade and the schemes you belong to — check them and write what they told you to do in the blank below. This sheet is the record that you did it, not the rule for when.

Write-on lines: Product · Lot / batch codes affected · Date codes affected · Reason for the action · Decided by · Date & time decided · Withdrawal or recall · Authority / scheme notified, and when · What they asked of us

Stock traced — every place the affected lots went, from your batch and sales records

Table, 6 rows: Customer / outlet / market · Qty supplied · Date supplied · Contacted (how / when) · Qty returned · Qty destroyed · Closed by

Cause and close-out

Write-on lines: Root cause found · Change made to stop it recurring

Signature strip: Closed by · Print name · Date closed

Questions

Why does the sheet not say when a recall is required?

Because that decision, and who has to be notified, is set by the rules where you trade and differs by country and product. The sheet has a blank for what you were told to do; it does not tell you.

What if we have never had to use it?

Print one and keep it with the batch records anyway. A recall is the one job nobody wants to design a form for on the day.
