The fillable versions have real form fields — open them in any PDF reader, type into them and save. Printing help.
Know who is in the building and when they left, which matters on a normal day for class numbers and matters a great deal on the day of an evacuation.
Small gyms, studios, martial arts and climbing clubs, and anyone running a staffed desk without a card system.
One page, top to bottom:
Printed under the title: A front-desk sheet for a gym, studio or club — who came in, when they arrived and left, what they came for, and their signature against it.
It holds personal details. A signed attendance sheet holds people's names and their movements, so keep it somewhere it cannot be read by the next person at the desk, and check the rules where you are on how long records like this may be kept and who may see them.
Write-on lines: Gym, studio or club · Site or room · Week beginning
Table, 9 rows: Date · Member name · Member no. · In · Out · Class or area · Signature
Ruled lines: Desk notes — guests, day passes, incidents, anything handed over
That depends on why you are keeping them and on the rules where you are, so no period is printed here. Decide the period deliberately, write it down, and destroy the sheets when you reach it rather than letting them pile up in a drawer.
Because a name written by someone else is not a record of attendance. The signature is what makes the line worth anything if it is ever looked at again.

Keep one page that answers who is a member, how to reach them and whether they have paid, instead of three lists that disagree.
Know who came, and have something to work from afterwards, without collecting more about people than the event actually needs.
Hold one client's history on one sheet, so you start each session knowing what you did last time and both of you know where the paid block has got to.
The same subject, with a version you can fill in on screen.
Which days you trained and what kind of session it was.
Every sheet, binder, game and puzzle on Upkeep Papers is free to download, with no email and no account. Making them is not free: each one takes days of research, design and checking, and the site costs money to run. If something here saved you time, you can buy us a coffee. It is entirely optional: nothing is locked, and nothing changes if you don’t.
