What this sheet is for
Take the information a yard needs at the point a horse arrives, rather than trying to find it during an emergency at eleven at night.
Who it is for
Livery yards, riding schools and anyone taking horses onto their premises.
How to use it
- Fill it in with the owner on the day the horse arrives.
- Get the emergency treatment consent question answered explicitly.
- Record the owner's third-party liability insurer.
- Update it annually — phone numbers and insurers change.
What is on the sheet
The layout is fixed at a size that fits both A4 and US Letter, so the A4 and Letter files are visually identical and print at full size on either. Rows are set at a height that an adult can actually write a date and a few words into with a pen — most printable tables are built in a word processor and never tested with one.

Is this a livery agreement?
No, and it should not be used as one. A livery agreement is a contract dealing with fees, notice, liability and insurance, and it is worth having drafted properly. This sheet records the practical information alongside it.
