What this sheet is for
Follow a prescribed rehabilitation programme exactly and record the response, so the vet's review is based on evidence rather than recollection.
Who it is for
Owners with a horse on box rest or controlled exercise, and yards managing one.
How to use it
- Write the vet's instructions in the box provided, in their words.
- Record what was actually done, including the days you did less.
- Note the response the same day.
- Take the sheet to the review appointment.
What is on the sheet
The layout is fixed at a size that fits both A4 and US Letter, so the A4 and Letter files are visually identical and print at full size on either. Rows are set at a height that an adult can actually write a date and a few words into with a pen — most printable tables are built in a word processor and never tested with one.
Questions
Does this tell me how to rehabilitate my horse?
No, and that is the point of it. Rehabilitation must be set by the treating vet for that specific injury, and exceeding it because the horse seems fine is one of the commonest ways injuries recur. The sheet holds their programme and records your compliance with it.
