Match a loose dog to a pitch in the dark, and have a signed record that the owner read your own site rules for pets.
Who it is for
Campsite, touring park and glamping wardens, and holiday parks that take pets on some pitches or in some units.
How to use it
- Fill a row as each party with an animal books in at the desk.
- Write the colour and markings, not just the breed — that is what identifies it.
- Note whatever paperwork you ask to see, and tick the column once you have seen it.
- Have the owner sign against your own pet rules before they go to pitch.
- Keep the sheet at the desk where anyone on duty can reach it.
What is on the sheet

Description, not just breed. The description is the part that earns its place: a loose dog on a site at dusk is found by its colour and markings, not its breed. Your own pet rules, any limit on numbers and whatever proof you ask to see are printed on your site's own paperwork, not here — this sheet records that the owner read them and signed.

Questions
Why does the sheet not say what vaccinations or paperwork are needed?
Because that depends on your site, your insurer and the rules where you are, and it changes. The column is a blank for whatever you ask to see, so the sheet still works when your requirements change.
Can this be used for animals other than dogs?
Yes. The columns are deliberately general — type and breed, colour and markings — so a cat in a motorhome or a parrot in a caravan records just as well.
