Who this is for

The teacher's record of a guided reading group. Each session takes a line: the date, the text and pages, who was present, what the focus was, who read aloud and what to pick up next time, with the group's members listed at the top.

Class teachers and teaching assistants running guided or group reading, reading volunteers with a regular group, and tutors who want a record of what a group has actually read and worked on.

What this helps you track

  • Which text and pages the group read at each session
  • Who was present, so a pupil who missed a session can be caught up
  • The focus of each session, in your own terms
  • Who read aloud, so it goes round the group fairly
  • Notes and next steps, written while you still remember them

How to use it

  1. Write the group name, the class, who leads it and the term at the top, and list the pupils in the group.
  2. Add a line for each session as it happens: date, text and pages, and who was there.
  3. Note the focus of the session in a few words: a skill, a question, a part of the text.
  4. Record who read aloud, so the next session starts with someone else.
  5. Write what to pick up next time before the group leaves the table.
  6. Start a new sheet when the group changes; the browser version saves the group list in your own browser.

Formats

  • Printable PDF (A4 and US Letter)
  • Fillable PDF with form fields
  • Fill in online, saved in your browser

The printable is laid out inside a box that fits both A4 and US Letter, so the two files are identical and neither one is scaled. The fillable PDF has real form fields. The online version keeps what you enter in your own browser and builds the completed PDF on your device.

How to print

Download the A4 or US Letter file to match the paper in your printer, open it in a PDF reader and print at 100% — not "fit to page", which shrinks the writing space. Both files leave a 14 mm margin, which clears the unprintable edge on home printers. To print the online version with your entries in it, use the Print this sheet button, which prints only the sheet and none of the page around it. More on printing.

Questions

How is this different from a reading record?

A reading record is kept per pupil, usually for home reading, and travels in the book bag. This is the teacher's record of a group session, on one sheet for the group.

Does it record reading levels or book bands?

No. Write the text and pages; if your school uses bands or levels, add them in the text column in your own scheme. Nothing is set here.

What goes in the focus column?

Whatever the session was for, in your own words: a comprehension question, a decoding skill, a discussion, fluency. It is a note for you, not a curriculum reference.

Can I use it for a book club or a reading volunteer?

Yes. The columns suit any regular group reading a shared text; who leads it and what the focus is are yours to fill in.
