Behaviour Incident Log
Free behaviour incident log — date, time, pupil, where, what happened, action, referral, parent informed and follow-up ticks, with counts. PDF, Excel, browser.
2 pages. The A4 and US Letter files are laid out identically, so neither is shrunk to fit.

Fillable PDF type and save
Spreadsheet formulas included
The fillable versions have real form fields — open them in any PDF reader, type into them and save. Printing help.
Fill it in online
The same sheet, on screen. Totals and figures are worked out as you type, rows can be added or removed, and you can print it or download the completed version as a PDF. Everything stays in your browser — your entries are kept in this browser's own storage on this device, nothing is uploaded, and there is no account.
Who this is for
A factual line for each incident. Date, time, pupil and place, what happened in plain words, what was done, who it was referred to and whether a parent was told, with a tick for the follow-up so the open ones stand out.
Class teachers, form tutors, heads of year and support staff who are asked to keep a written account of incidents, and anyone who has had to reconstruct a term from memory for a meeting.
What this helps you track
- When and where each incident happened, and who was involved
- What happened, written as fact rather than opinion
- The action taken at the time and who it was referred to
- Whether a parent or carer was informed and whether the follow-up is done
- How many incidents are on the sheet and how many are still open
How to use it
- Write the class or group, the term and your name at the top.
- Add a line as soon as you can after an incident: date, time, pupil and where it happened.
- Describe what happened in the words you would use to a colleague who was not there, without judgement of the pupil.
- Record the action you took and who, if anyone, it was referred to.
- Tick parent informed when that has actually happened, and follow-up done when whatever your school's process asks for is complete.
- The summary counts incidents, parents informed and open follow-ups in the browser version and the workbook.
Formats

- Excel / Google Sheets workbook
The printable is laid out inside a box that fits both A4 and US Letter, so the two files are identical and neither one is scaled. The fillable PDF has real form fields. The workbook uses plain formulas only — SUM, IF, COUNTIF and the like — so it opens in Excel, LibreOffice and Google Sheets without anything breaking, and every calculated cell is grey so you can see which figures it worked out.
How to print
Download the A4 or US Letter file to match the paper in your printer, open it in a PDF reader and print at 100% — not "fit to page", which shrinks the writing space. Both files leave a 14 mm margin, which clears the unprintable edge on home printers. To print the online version with your entries in it, use the Print this sheet button, which prints only the sheet and none of the page around it. More on printing.
Questions
Does the sheet say what counts as an incident?
No. Your school's behaviour policy decides what is recorded and what happens next. The sheet gives you somewhere to record it.
Why the emphasis on factual wording?
Because the log may be read later by a parent, a manager or someone outside the school. What was seen and heard holds up; interpretation does not.
Should I keep one log per pupil or per class?
Either works. A class log shows patterns across a group; a per-pupil log is easier to take to a meeting. Sort the workbook by pupil if you keep one sheet.
What does open follow-up mean?
A line where the follow-up box is not yet ticked. The summary counts them so nothing is left unfinished at the end of the week.
