What this sheet is for
Give every piece of propping, shoring, falsework or edge protection an owner and four dated signatures, so nothing is loaded or struck on an assumption.
Who it is for
Site managers, temporary works coordinators, small builders and anyone propping, shoring or supporting something during construction.
How to use it
- Register the item before it is installed, not afterwards.
- Record who designed it and, separately, who checked the design.
- Nothing gets loaded until the permit-to-load line is signed.
- Nothing gets struck until the permit-to-remove line is signed.
- Review the open items at every site meeting.

Every item has an owner and four signatures. Temporary works fail when nobody owns them. This register records who designed each item, who checked the design, who signed it off for loading and who signed it off for removal. It sets no design standard and prints no loads. Who may authorise and carry out this work, what training and certification they need, what must be recorded and how long it must be kept differ by country and by workplace. This sheet names no scheme, sets no criteria and satisfies no regulation on its own — check what applies where you are.
